Title: Clinician Duration: One year with possible renewal Application deadline: 1st February 2013 Duty station: Mbale Regional Referral Hospital Program: National TB and Leprosy Control Program (NTLP), Ministry of Health-Uganda.
GOAL:
The goal of this fellowship is to provide a training-through-service opportunity for a clinician. Through this experience, the Fellow will provide direct technical assistance to the National TB and Leprosy Control Program (NTLP) of the Ministry of Health-Uganda for strengthening the quality of TB diagnosis, case management, infection control, laboratory/clinical interface and other aspects of TB control activities at the Regional Referral Hospital in Mbale and the associated 14 districts of Eastern Region.
OBJECTIVES:
Support the Mbale Regional Referral Hospital and surrounding districts of Eastern Region (14 districts)to (1) improve the detection and case management, (2) enhance the quality and use of patient registers for decision-making, (3) Extend enhanced capabilities in TB detection, case management, and infection control to other clinical cadres, and (4) support the operations research agenda of the NTLP.
MAJOR RESPONSIBILITIES:
Over the period of performance, the incumbent will receive advanced training in TB control epidemiology and interventions, under the mentorship and technical supervision of the Program Manager, NTLP. Additional technical assistance will be provided by the Africa Field Epidemiology Network (AFENET), U.S. CDC-Uganda, U.S. CDC Atlanta, and other MOH technical partners. The incumbent will sit within the NTLP of the MOH, based at the Mbale Regional Referral Hospital Department of Medicine, liaising with key partners, and supporting implementation of the following activities:
  1. TB patient care and management at the Mbale RRH
  2. Implementing and refining algorithms for detection and treatment of pediatric TB
  3. Building capacity among health workers to prevent and treat MDR- TB
  4. Strengthening data quality at the laboratory-clinical interface and connectivity with patient follow-up in districts and communities.
  5. Engage in special studies and operations research based on TB program priorities – such as; pediatric TB prevalence and treatment, improving case detection among HIV+ patients, strengthening patient follow-up and cure rates; evaluating MDR-TB diagnosis and treatment at regional referral hospitals, among others.
  6. Support cascading skill sets development among nurses, clinical officers, and other relevant clinical cadres through both training and on-the-job supportive supervision for:
    •  Analysis and use of data for decision-making
    • Improved adherence to diagnosis guidelines
    • Infection control practices
    • TB and TB/HIV co-infection case management
    • TB Contact tracing activities
    • TB/HIV Prevention
    • Intensified TB case finding among HIV positive clients
    • Program management (continuous quality improvement, planning, supervision, etc.)
  7. Develop, test and refine SOPs and guidelines for operationally linking regional referral hospital support to TB zones and related districts for quality assurance, technical support, etc, which can inform national scale-up
  8. Development of quality improvement interventions to address specific challenges identified during supportive supervision, together with the regional Medical Officer Epidemiologist for TB Control and the laboratory specialist
Specific outputs include the following:
  • Monthly and quarterly program reports
  • Data quality assessments done in the RRH
  • Trainings in TB detection, case management, and infection control tor the regional referral hospital and 14 districts
  • At least one special study protocol developed and approved by relevant authorities (year 1); implementation of study protocol (year 2)

Requirement:

  • Medical degree.
  • Strong analytic skills.
  • Previous experience in training other health workers.
  • At least two years' experience in TB clinical practice.
