Medical Records Clerk

Position Summary: The Medical Records Clerk ensures the overall completeness of patient records and manages patients’ records at the Centers of Excellence. This person also performs statistical analysis of all patients' medical records while ensuring quality in indexing and develops a system that monitors all client records that is consistent with the COEs requirements.
Duties and Responsibilities
· Maintain patient files and retrieve files for scheduled appointments
· File all patient data upon receipt of information
· Initiate records fornew patients and create file labels/computer index for each patient
· Maintain filing statistics and audit filing sequence.
· Develop a system that monitors all client records that is consistent with the requirements of the COE
· Initiate the medical chart and route the chart to the appropriate personnel.
· Insert any lab results or other results needed for review by the physicians as per procedures established by COE or as instructed by the clinical staff.
· Check for accurate patient identifying information on all documents
· Verify and update all patient identifying information if necessary.
· Pull patient charts as needed for special audits, review and refill requests for audits as appropriate
· Prepare and mail out requests for records when presented with a properly completed medical release.
· Perform any other relevant duties as required
Qualifications and Experience:
· High school graduate or equivalent
· At least two years of specialized training and/or experience in keeping medical records
· Familiar with health care centers setting and commitment to serving patients.
· Ability to work with people from different backgrounds and work under pressure.
· Excellent written and oral communication skills both in English and Swahili.
· Familiar with medical terminology and able to understand it.
· Possess excellent customer service skills.
· Well organized self-starter.
· Ability to maintain confidentiality
· Proficiency with computers is essential
· Have the ability to multitask and prioritize tasks
